Call your agent or the customer service number on your ID card. BCBS has many different plans with differing levels of rehab benefits so you’ll first want to make sure your plan has the coverage you need. If it does the customer service people will also be able to give you a list of facilities that are in network.

Be aware that even if a facility is not covered by BCBS some plans will allow you to get care anyway. If you have a BCBS PPO plan you can go outside of the network but your costs will be higher. If you have a BCBS HMO plan you cannot go outside of the network.
